Your First Consultation with a Fertility Doctor: What to Expect

Deciding to see a fertility specialist is a big step — and for many people, it comes after months or even years of trying, hoping, and wondering. Walking into that first appointment can feel nerve-wracking, even when you know it’s the right move. The good news? Knowing what to expect can make the whole experience feel a lot less overwhelming.

Here’s a warm, honest look at what typically happens at your first fertility consultation.

Before You Go: What to Bring

Most fertility clinics will ask you to fill out intake paperwork ahead of time, but it’s also helpful to come prepared with a few things:

  • A summary of your menstrual cycle history — how regular your cycles are, the length, and any irregularities you’ve noticed.
  • Any prior testing results — bloodwork, ultrasounds, semen analyses, or records from previous pregnancies, surgeries, or fertility treatments you may have previously undergone.
  • A list of current medications and supplements.
  • Your questions. (More on this below.)

If you have a partner joining you, encourage them to come — especially if a semen analysis may be part of the workup. Fertility is rarely a one-sided conversation.

The Consultation: What You’ll Talk About

Your first appointment is primarily a conversation. The doctor will want to understand your full picture before recommending any next steps. Expect questions about:

  • How long you’ve been trying to conceive
  • Your menstrual cycle and any irregularities
  • Previous pregnancies, miscarriages, or fertility treatments
  • Your general health history, including surgeries, chronic conditions, or prior STIs
  • Family history that might be relevant (such as early menopause or genetic conditions)
  • Lifestyle factors like stress, diet, and exercise

This isn’t an interrogation — it’s the doctor building a complete picture of your health so they can give you the most thoughtful guidance possible. Be as honest and thorough as you can. Nothing you share will be surprising to a fertility specialist, and everything is relevant.

The Pelvic Ultrasound

After the conversation, your doctor will likely order a transvaginal pelvic ultrasound. This allows them to assess your uterus and ovaries — looking at the shape of the uterus, checking for fibroids or ovarian cysts, and counting your antral follicles (small resting follicles that give an estimate of your ovarian reserve).

This ultrasound is usually quick and essentially painless. It gives the doctor a real-time snapshot of your reproductive anatomy that’s incredibly valuable in understanding your situation.

Bloodwork and Hormonal Testing

You may have blood drawn at this first visit, or your doctor may schedule it for a specific day in your menstrual cycle, namely cycle day 3 (or 4). Common tests include:

  • FSH (follicle-stimulating hormone) and estradiol: Measured early in your cycle to assess ovarian reserve.
  • AMH (anti-Müllerian hormone): A marker of your egg supply and quality which can be tested any time in your cycle.
  • TSH (thyroid-stimulating hormone): Thyroid issues can affect fertility.
  • Prolactin: And sometimes androgen (“male hormone”) levels, depending on your history.

If you’re bringing a male partner, a semen analysis is a standard and essential part of the evaluation. About 40–50% of fertility challenges involve male factors, and it’s one of the most straightforward tests in the workup.

What Happens Next

By the end of your first visit, you may not have a definitive diagnosis — and that’s okay. Often, the doctor will want to review all your test results together before making a plan. You’ll typically schedule a follow-up to go over findings and discuss options. That second conversation is usually where a more concrete path forward begins to take shape.

Some patients leave their first appointment with a clear diagnosis. Others leave with more questions and a plan to gather more information. Either way, you’ll leave knowing more than when you walked in — and that matters.

Questions Worth Asking

Don’t be shy about speaking up. Good fertility doctors expect and welcome questions. A few worth considering:

  • What tests do you recommend, and what are we looking for with each one?
  • How long will it take to have a full picture of what’s going on?
  • Based on what you know so far, what might be contributing to our difficulty conceiving?
  • What are the general treatment options we might be looking at?
  • What does the next few months of this process look like?
